The Palisade Radiofrequency Thermocoagulation Technique for Low Back Pain

Abstract
Palisade radiofrequency thermocoagulation (RFTC) is an alternative procedure of placing multiple RF cannulae perpendicular to the dorsal sacrum and producing overlapping bipolar RF lesions between the dorsal sacral foramina and the ipsilateral sacroiliac joint (SIJ). Compared to conventional RFTC, the "strip lesion" connecting two cannulae tips was expected to destroy sacral lateral branch nerves irrespective of irregular path to reach the SIJ. A 59-year-old woman visited our pain clinic with a 3-year-history of low back pain (5/10 on a VAS) with referred pain to lateral aspect of right thigh and knee. The symptom was relieved after L4, 5 medial branch block and SIJ block. The palisade RFTC was applied to right L4, 5 dorsal rami and S1-S3 lateral branches. After 1 month, she presented 2/10 on a VAS, and caudal epidural block was done. Another one month later, her pain was completely subsided.
